Your EAP is a totally confidential programme. We work under the same laws of the land, in terms of Privacy, that your Doctor and your Lawyer do. This means that your identity, the fact that you have visited us and the nature of your issue must remain confidential.
Except in situations of concern regarding self harm or harm to others, or criminal activity, we can only mention your name or your issue to someone else, with your written permission. This may be your wish if your issue has a workplace component to it and you'd like us to raise the issue, with your organisation, on your behalf.
To assist us maintain the appropriate level of privacy we have created the "Authorisation for Disclosure of Information" form. You can print this form from this website, or your counsellor can give you a copy and help you complete it.
